    *During assessment gather objective data , such as an open incision or a reduced caloric intake, and subjective data, such as client’s complaint of tenderness over a surgical wound site .Then review the data carefully, looking for clusters of characteristics or risk factors that create a pattern.
 Examples of nursing diagnosis :
*Risk for infection.
*Imbalanced nutrition ,less than body requirements.
*Impaired oral mucus membrane
*Risk for impaired skin integrity.
*Social isolation.

*Impaired tissue integrity

HYPERSENSITIVITY Types of Oral Hypersensitivity Reactions | Immunological and Inflammatory ⁃ Immunologic ally induced inflammation of lungs of a susceptible  host caused by repeated inhalation of a variety of substances including organic dusts. ⁃ Included are molds and other fungi from sources such as cheese ,vegetables,mushrooms, flour ,bark of trees detergents,and contaminated humidification systems. ⁃ In the stage , patients may present with cough , fever, chills, malaise and shortness of breath . ⁃ In sub acute and chronic forms, the onset of symptoms is gradual and prolonged. ⁃ Treatment include identifying abs avoiding causative agents.
